Outreach

The Outreach service started during the lockdown when the Day Centre was required to suspend the day care provision. We realised that older residents were at risk of being more isolated than ever, so we quickly responded with a service that would offer companionship as well as support with essential everyday activities. 

 

Ideal for individuals who need a little companionship and/or support in the home. Book one of our outreach workers to come and visit you at home for;

  • companionship/befriending

  • conversation

  • play board games

  • hobbies/crafts

  • help with light duties; eg. washing up, vacuuming, making a snack

  • prompting with medication etc.

 

This service is tailored to individual need on days and times that you need. Please note: We are not able to administer medication or provide personal care in the home.

Fees

Outreach visits cost £22.45 per hour based on a minimum one hour booking.

Case study (Names have been changed to protect individuals' identity)

Agnes lived alone, with no family available in the local area. She utilised the shopping and meal delivery services for a few a couple of years. This was enough to help her stay independent for some time before her neighbour enquired about Agnes coming to the Centre for a day out. The neighbour had noticed that Agnes' memory was not as good as it had been and that she might not be coping as well as before. She suggested that some support and stimulation would be beneficial.

 

We arranged with Agnes to come to the Centre one day per week where she had a meal with friends, chatted and was able to take part in craft activities that she enjoyed. She got a lot out of her day at the Centre but increasingly was finding her time at home very lonely. Agnes thought it would be nice to have a visit from one of our outreach workers, who she already knew from coming to the Centre.

 

We arranged for an outreach visit twice a week to chat with Agnes, support her with small tasks around the home like washing up, putting away shopping and checking food labels in the fridge. We were able to provide a joined up care package to meet Agnes' needs so that she was engaged and supported for most days of the week. This also allowed us to monitor her wellbeing and liase with other care professionals, GP and family members to ensure that Agnes was able to access the support and services she needed, even when she was no longer able to do this for herself.
