The Attleborough Day Centre service users: those individuals that have a service provided to them by Attleborough Day Centre General Data Protection Regulations 2018: How we use our service users’ information We process personal data relating to the individuals we provide a service to. We do this is in order to be able to provide a service that meet their individual requirements.
The personal data we hold includes identifiers such as Name, DOB, address, contact phone numbers, Next of kin, hospital and social services identity numbers, GP contact, medical conditions, support plans, contracts and invoices (for privately funded service users). We have a contractual basis for processing this information in order to be able to provide a service to users and retain this information for six years after the service is terminated and contract ceases.
Personal Information may also include personal sensitive information such as sexuality, race, your religion or beliefs, and whether you have a disability, allergies or health conditions. It is important for us to have a complete picture, as this information assists staff involved in your care to deliver and provide improved care, deliver appropriate treatment and care plans, to meet your needs.
We will not share information about users with third parties without the individuals consent unless the law allows or requires us to. We are required to share some of users’ personal data with:
·Norfolk social services for contractual purposes
Others we may share an individuals’ personal data with:
·Next of kin/relatives (where appropriate) ·GPs and other health professionals involved in your care
Service users have the right to complain to the Information Commissioning Office if they believe there is a problem with the way that their information is being handled at Attleborough Day Centre.
